New Patient Registration

All patients applying to register must provide proof of entitlement to NHS treatment. Please ask reception for details. Eligible patients will then be asked to complete a GMS 1 form and a short health questionnaire.

New partients are required to see HCA/Practice Nurse as part of registration where a new patient health check will be completed if you are aged 40-74 you will be offered NHS healthcheck (Click on the  NHS Health Check  tab above).

Medical treatment is available from the date of registration. Please contact reception for further information.
